Who to Refer: We specialize in the assessment of mild cognitive impairment and early-stage dementia. However, we also welcome general adult neuropsychological referrals for patients with other neurodegenerative, neuromedical, and neuropsychiatric conditions. We additionally offer preventative evaluations for patients interested in long-term brain health. We do not accept pediatric referrals. Given our services are offered via telehealth, patients must have access to a computer with video/microphone and be able to use this technology independently or with the assistance of an available caregiver.

What to Expect: We will contact your office to confirm when your referred patient has scheduled an appointment and update you if we are unable to contact your patient or if they cancel their appointment. We will send you a written neuropsychological report within two weeks of the patient’s final appointment date, pending any unique time limitations. Please let us know your preferred method for receiving the evaluation report in the referral form.

Availability: We will provide referring providers with ongoing updates on availability as relevant. If we do not have any appointments available within a reasonable timeframe, we may recommend alternative evaluation options.
